Withdraw from the MTBP

You can withdraw from the Mass Transportation Benefit Program (MTBP) using the automated application .

If you do not have a CAC or access to a .mil computer, please follow the steps below to withdraw from the program:

  1. Access the paper application  and check the box next to "Withdrawing."
  2. Fill in your last name, first name, middle initial, and the last 4 digits of your social security number.
  3. Print the form and fax it to 703-697-2144.
