Sick Leave for Personal UseAppropriate UseAn employee may use sick leave for personal use

Requesting Sick LeaveUnless otherwise instructed by your supervisor, leave requests for planned medical appointments must be submitted in advance to your supervisor on a Form SF-71. For unexpected illness or emergencies, employees should call to request leave as soon as practical. If possible, this should occur before the start of your shift to allow management to make arrangements in the office necessitated by your absence as soon as possible. Medical DocumentationIf you are requested to, or choose to, bring in medical documentation to support your use of sick leave, you may complete the following form: Form WH-380.
