Sick Leave for Personal Use

Appropriate Use

An employee may use sick leave for personal use

  • to receive medical, dental, or optical examination or treatment;
  • because he/she is incapacitated for the performance of duties by physical or mental illness, injury, pregnancy, or childbirth;
  • because he/she would, as determined by the health authorities having jurisdiction or by a health care provider, jeopardize the health of others by his or her presence on the job because of exposure to a communicable disease; or
  • because he/she must be absent from duty for purposes relating to the adoption of a child, including appointments with adoption agencies, social workers, and attorneys; court proceedings; required travel; and any other activities necessary to allow the adoption to proceed.

Requesting Sick Leave

Unless otherwise instructed by your supervisor, leave requests for planned medical appointments must be submitted in advance to your supervisor on a Form SF-71.

For unexpected illness or emergencies, employees should call to request leave as soon as practical. If possible, this should occur before the start of your shift to allow management to make arrangements in the office necessitated by your absence as soon as possible.

Medical Documentation

If you are requested to, or choose to, bring in medical documentation to support your use of sick leave, you may complete the following form: Form WH-380.
